To help make your visit smoother, please complete the appropriate patient forms before your appointment. New patients should complete the new patient forms listed below. Established patients should complete the welcome back form and financial policy before their next visit.
Crystal Eyecare is a small, appointment-based practice, and each reserved appointment time affects our ability to care for other patients. If you need to reschedule or cancel your appointment, please provide at least 24 hours’ notice so we can offer that time to another patient.
Appointments canceled with less than 24 hours’ notice, or missed without notice, may be subject to a $50 cancellation fee. We may ask for a credit card to keep securely on file. Your card will not be charged unless we have spoken with you about the missed appointment or have attempted to contact you for one week without success.
We appreciate your understanding and cooperation. Our goal is always to see you for your scheduled visit and provide the attentive care you deserve.
